To protect our customers, we require signed notification of termination of service using form #1817 U4 Disconnect Switchback or Social Security confirmation before services are cancelled to make sure that termination occurs when and how you want it to. This form can be completed in person or by calling Customer Service during regular business hours. There is a fee for this action and some Pay-Per-Use and Long Distance charges can take up to 2 bill cycles to appear on your bill.
