To protect our customers, we require signed notification of termination of service, using form #1817 U4 Disconnect Switchback, before services are cancelled to make sure that termination occurs when and how you want it to. This form can be completed in person or by calling Customer Service during regular business hours.
Please be aware that this can only be accomplished with a signature by an authorized person on the account. There is a charge for this action. Some Pay-Per-Use and Long Distance charges can take up to 2 bill cycles to appear on your bill.
